Confidentiality

With the exception of the specific instances below, all personal information gathered during the provision of psychological services will remain strictly Confidential and secure. Exceptions are:

A. It is subpoenaed by a court; or

B. Failure to disclose the information would place you or another person at risk; or

C. Your prior approval has been obtained to provide a written report to another professional or agency. Please note that I do not provide reports for the purpose of legal proceedings and if you are referred under Medicare then this initiative requires that your psychologist provides regular brief progress reports to the referring practitioner; or

D. Your prior approval has been obtained to discuss the material with another person. In order to provide the best possible client care, your case at times will be discussed with other psychologists. In such instances you will not be personally identified.
